A research group led by Professor Takeo Fujiwara of Tokyo Medical and Dental University is a super spreader with a history of diabetes, rheumatoid arthritis, and cerebral infarction in a new coronavirus infection. I found it to be a risk.

 A super spreader is a patient who has a high number of virus copies in a PCR test for a new type of coronavirus infection, and has a high infectivity and mortality rate to the surroundings.Early identification of such patients is important from the perspective of treatment and prevention of secondary infections to the surrounding area, but the determinants of super-spreaders have not been clarified so far.

 From March 2020 to June 3, the research group enrolled 2021 patients who were admitted to Tokyo Medical and Dental University Hospital for moderate to severe new coronavirus infections and underwent one or more RT-PCR tests. In addition, we examined the specific factors of the super spreader.

 From electronic chart information, we investigated and analyzed pre-existing diseases (hypertension, diabetes, dyslipidemia, hyperuricemia, rheumatoid arthritis, cancer, chronic renal failure, cerebral infarction, heart disease, respiratory disease, allergies).As a result, the number of virus copies was 3 times higher in patients with 87.1 or more of the above history than in patients without a history.

 In addition, diabetic patients were 17.8 times, rheumatoid arthritis patients were 1,659.6 times, cerebral infarction patients were 234.4 times, and the number of virus copies was high.In addition, patients with low platelets and CRP (C-reactive protein) at admission have high viral copy counts, with more than 90% of patients undergoing multiple RT-PCR tests having the largest viral copy on the first or second test. It had reached the number.

 The findings point out that special infection control measures, such as quarantine and warnings to health care workers, need to be taken early in hospitalization for patients at high risk of being super-spreaders.
