A joint research group of Tokyo Medical and Dental University and Keio University conducted a questionnaire survey to understand the status of treatment explanations and consent acquisition for patients and their families by medical personnel who treat new coronavirus infections.

 When using a ventilator or extracorporeal membrane oxygenation device (ECMO) for a severely ill person with a new coronavirus infection, it becomes difficult to communicate with the patient and family due to the unconsciousness of the drug and the need for isolation.Treatment with the understanding of the patient and family requires explanation of the treatment and understanding of the actual situation of obtaining consent.

 そこで今回、インターネットによるアンケート調査(2020年12月1日~2021年1月17日)を実施。対象は「ECMOネットワーク37施設」(国内ECMO所有医療機関の有志ネットワーク)所属の医師37名(各施設1名)で、うち15名(15施設)より回答を得た(回答率40.5%)。

 33.3% of the facilities have created guidelines for the installation of ventilators and ECMOs on patients.100.0%, 75.0%, and 50.0% of the facilities had the items of adaptation standard, withdrawal standard, and discontinuation standard for both devices as guidelines, respectively.

 83.3% of the respondents answered that there were other attendees than the patient in the doctor's explanation.The attendees were 75.0% of nurses, 58.3% of families, and 41.7% of other doctors.In addition, 83.3% of the respondents gave oral explanations and obtained consent.

 91.7% of respondents explained to patients and 90.9% to their families about the availability of ventilators in severe cases.83.3% explained to patients and 100.0% to their families about the possibility of using ECMO, which is a heavy burden on the body.

 From the results of the survey, it was shown that the treatment was explained and consent was obtained remotely and orally, instead of face-to-face written consent.Create an environment where digital devices (PCs, tablets, etc.) can be used, create guidelines such as ECMO, and obtain explanations and consent based on them, in order to confirm faces and situations among related parties and explain in documents and figures. Is desirable.
