While allogeneic vascular cell transplantation is a treatment that can be expected to cure relapsed / refractory leukocytes, HLA (Human Leukocyte Antigen) is used to reduce the risk of immunological complications in about half of patients. No matching donor can be found at the right time.

 Research groups such as Kyoto University, The Japanese Data Center for Hematopoietic Cell Transplantation, Eurocode, the European Cord Blood Research Facility, the European Society for Blood Bone Marrow Transplantation, and the Japanese Data Center for Hematopoietic Cell Transplantation have joined forces to rapidly expand umbilical cord as an alternative source of transplantation to HLA-matched donors. The first international collaborative study between Japan and Europe for blood transplantation revealed race-independent prognostic factors.

 A total of more than 1000 cases of cord blood transplantation are performed annually in Japan, and it has been shown that the results are almost the same as those of HLA-matched unrelated bone marrow transplantation.However, the problem is that the frequency of engraftment failure and transplantation-related complications is slightly higher than that of other transplantation sources, and elucidation of prognostic predictors is desired.

 This study enrolled 3764 patients from Japan and 1027 patients from Europe, and was the first international collaborative study to find a universal prognostic predictor regardless of race.As a result, despite the fact that the patient background and treatment content differed greatly between Japan and Europe, the newer the transplant year, the younger the patient, and the remission at the time of transplantation, the lower the risk of leukemia recurrence in both Japan and Europe. It was found that the effects of each factor on transplantation results are very similar, such as the higher the survival rate.In other words, it can be said that there is no significant difference in prognostic factors between Japan and Europe, indicating that integrated analysis is possible.

 The results of this research are considered to be important basic data for accelerating joint research between Japan and Europe.Based on this data, the group plans to plan joint retrospective studies and intervention trials between Japan and Europe.
